Request for Records (GRAMA)

Fire Incidents Report
Fire Incident reports may be requested by completing a records request form.

Medical Incident Report
Records containing protected health information as defined in 45 C.F.R., Part 164, are governed by HIPAA (the Health Insurance Portability Act) and are not subject to GRAMA.

Environmental Records Requests
Record requests for environmental property assessments, directly associated with fire code violations, hazardous material spills, etc. can be obtained by completing a GRAMA request form.
